The documentation submitted must have the following:

• Name exactly as indicated in the form.
• Photograph as given in the form.
• For Joint Account Holders the below mentioned documents are required for both, applicant as well as joint applicant(s) in Bank and Demat.
• If the joint applicant(s) in Bank is/are blood relative(s)/spouse of the applicant then declaration given on page 14 of the form has to be filled.
• The document provided as residence proof should bear address of the applicant as given in the form.
• Please note that documentation other than as specified below will NOT be accepted.

1). PAN Card with Photograph.(In case the name on the Pan Card mismatches with the name on the application form and the Photograph on the PAN Card please submit an additional proof such as Passport, Voter’Id, Driving License or Mapin Card)

1. Bank Passbook / Bankers verification letter (only from ICICI Bank)/Bank statement
2. Ration Card with Photograph
3. Passport
4. Leave and license agreement along with utility bill in the name of landlord
5. Telephone Bill (2 months old)
6. Electricity Bill(2 months old)
